Even after log in we don't see the ADD button anymore - this site has been up for nearly two years.

Host just upgraded yesterday - does that mean anything ???

I would assume the upgrade is to blame, otherwise it's highly coincidental... Start by looking at the error logs, if available, to see what all is going wrong.
